Enrolment Procedure and Requirements

 Start of Enrollment for Senior High, Junior High and Grade School

Secure first an admission form at the Guidance office.

  1. Pay the testing fee of P150.00 for Grades 1 – 6 and Interview fee of P100.00 for Kinder and Preparatory.
  2. Proceed again to the Guidance office to present the receipt then take the admission test.
  3. Interpretation of test result.
  4. Interview by the Principal.
  5. Return to the Guidance office for the over-all admission result (OAAR).
  6. The examiner must fill out the student’s profile and enrollment form.
  7. Submit 1 recent 1 x 1 ID picture and long size folder.
  8. Enrollment at the Registrar’s office.
  1. Photocopy of Birth Certificate or Baptismal Certificate
  2. Photocopy of Report Card (Form 138)
  3. Certificate of Good Moral Character (For Transferee’s, Grade II – VI)
  4. 4 Copies of 1×1 Picture
  5. 1 Long Folder
  6. Testing Fee Receipts from the Cashier’s Office (Php 100.00)

New Students:

  1. Applicants are advised to secure the High School Entrance Test Application Form (HSETAF) and Student’s Profile Form (SPF) at the High School Cashier.
  2. Pay the non-refundable application fee worth Php150.00.
  3. After accomplishing the for High School Entrance Test Application Form (HSETAF) and Student’s Profile Form (SPF), proceed to the Admissions, Guidance and Testing Center to submit the following documents:
    • 1 pc. long white folder;
    • 2 pcs. 1×1 ID picture with blue background attached to HSETAF and SPF;
    • Authenticated photocopy of Report Card with general weighted average in the Final Grading;
    • Authenticated photocopy of Good Moral Certificate; and,
    • HSET official receipt.
  4. Get the test permit from the Admissions, Guidance and Testing Center.
  5. Take the High School Entrance Test on the scheduled date.
    • January-March 2015: All Saturdays 8:30am-9:30am
    • April-May 2015: Mondays- Fridays 8:30am-9:30am and 1:15pm-2:15pm
    • Saturdays 8:30am-9:30am
  6. Test Result will be released an hour after the examination and followed by the Oral Reading Ability Assessment conducted by the AGTC Staff.
  7. Qualified examinees will be interviewed by the High School Principal.
  8. Return to the High School Admissions, Guidance and Testing Center to get the Overall Admission Result (OAR). Take note, OAR comprises the rating from the entrance test, report card, and principal’s interview.
  9. Proceed to the Registrar’s Office for enrolment.

Transferees:

  1. Applicants are advised to present the necessary documents to the High School Principal for initial evaluation. Below are the document needed:
    • Report Card
    • Good Moral Certificate
    • Accomplished *Recommendation Form either from his/her former class adviser, guidance counselor, or principal.
  2. Get the Entrance Test Recommendation (ETR) from the principal confirming and recommending you to take the entrance test in Divine Word College of Legazpi- High School Department.
  3. Secure the High School Entrance Test Application Form (HSETAF) and Student’s Profile Form (SPF) at the High School Cashier.
  4. Pay the non-refundable application fee worth Php150.00.

  • NEW STUDENTS (TRANSFEREES)
    1. INFORMATION BOOTH – distribute checklist of admission requirements and enrolment procedure.
    2. CASHIER – Pay the testing fee.
    3. Admission, Guidance & Testing Center – For College Admission Test and its result, encodes names of applicants, interview, filing up of Personal data sheet.
    4. DEANS – Final Interview, evaluation of subjects, approves admission on the chosen course, and appropriate curriculum.
    5. REGISTRAR – Reviews the credentials & assign ID Number.
    6. CASHIER – The enrollee pays the required initial payment.
    7. ICTC – Picture taking, crafting and printing of School ID
    8. DEAN’S OFFICE – Encodes his/her schedule of classes.
    9. REGISTRAR – The enrollee submits the credential and secures a validated print-out of schedule of classes.
    10. MEDICAL/DENTAL – The enrollee goes to the Medical/Dental Clinic for Physical Check up.
    11. SAO – The enrollee goes to the Student Affairs Office to claim the School ID.
  • OLD STUDENTS
    1. INFORMATION BOOTH – distribute checklist of admission requirements and enrolment procedure.
    2. CASHIER – Pay the testing fee.
    3. DEANS – Final Interview, evaluation of subjects, approves admission on the chosen course, and appropriate curriculum.
    4. REGISTRAR – The enrollee submits the updated slip from AGTC in exchange of the validated print-out of his/her schedule of classes to be presented during the 1st day of classes.
    5. MEDICAL/DENTAL – for Physical Check up.
    6. SAO – The enrollee goes to the Student Affairs Office for validation of School ID
  1. Duly accomplished registration form(Available at the AGTC)
  2. NCAE Results (Photocopy)
  3. NSO Birth Certificate
  4. Certicate of Good Moral Character
  5. Grade 10 Report Card/ Latest Quaterly Grade (Photocopy)
  6. 1 long White Folder
  7. 2pcs 2×2 I.D picture with white background.
  8. Sharpened pencils.
  9. Php 200.00 Testing fee
